Health Department

The Norwood Health Department was founded in 1890 and was originally located in the basement of the City Municipal Building. Because of the growing population and the need for expanded services and space, the City of Norwood constructed the building located at 2059 Sherman Avenue. Many residents remember receiving oral polio sugar cubes and vaccinations at the health department. We are mindful of the sacrifices that those before us made in bringing public health to the city.

Birth Certificate Form

Congratulations on having a child! Please fill out the Birth Certificate Form (PDF) and return it to the health department. Certified copies of birth certificates at the Norwood City Health Department are $25 each. This cost includes fees that we are required by law to collect on behalf of other agencies ($1.50 for Family Violence Prevention; $3 for Child Abuse Prevention; $5 for the Ohio Department of Health). The Norwood City Health Department retains only $7.50 of the cost to maintain records, purchase vital statistics paper and supplies and issue records. Each local health district sets their fees for vital records storage and maintenance. Payment for vital records at the Norwood City Health Department must be made by cash, money order or credit card.

Food Plan Review Application

Needed if conducting new construction and new food establishment in an existing building, remodeling of existing licensed food facility (New Finishes, New Equipment, Changes to Food Prep Area), and reopening a closed food establishment. View or download the Food Plan Review Application (PDF).

Mobile Food Unit Plan Review Packet

A mobile food license is required for a food service operation or a retail food establishment that is operated from a movable vehicle or portable structure that is able change location. Fill out and return the Mobile Food Unit Plan Review Packet (PDF) to apply for a mobile food license.

Temporary Food License Application

A temporary food license is required for any individual that wishes to prepare and/or serve food for a cost or required donation at an event for no more than 5 consecutive days. You are limited to 10 temporary food service licenses per person, per licensing period (March 1st through the Last Day of February). In order to receive a temporary food license, fill out and return the Temporary Food License Application (PDF).
